Welcome to Stengler Center for Integrative Medicine. We look forward to meeting you! If you are a new patient and have already scheduled an appointment, please take time to complete our new patient registration form before your appointment. This form is very important for Dr. Stengler in analyzing your health history and making recommendations that treat the root cause of your illness. All information will be kept strictly confidential and will become part of your medical record.

Note: Please allow some time to complete the form. While Dr. Stengler prefers to collect as much information as possible, only the first two pages are mandatory. The form is completed online on our secure server. Please let us know if you have any questions or concerns.
